Willful and serious safety violations were responsible for an Illinois grain elevator explosion that claimed five lives, the Occupational Safety and Health Administration said. OSHA proposed fines of $14,000 against ADM-Growmark Inc. because it either knew or should have known about deficiencies in the grain-dust collection system at its elevator in Joliet, said Ronald J. McCann, the director of the agency’s office in Calumet City, Ill. Five people were killed and three were injured in an April 22 blast at the elevator complex, situated along the Des Plaines River 40 miles southwest of Chicago.
